Wellington Kebab & Pizza, a takeaway at 2 North Street, Wellington, Somerset was given the score after assessment on April 16, the Food Standards Agency's website shows.
It means that of Somerset's 341 takeaways with ratings, 267 (78%) have ratings of five and just one has a zero rating.
